Always perform a Blocking Verification Test after using this product. De-Energisation Test: All associated isolation devices, such as switches, valves, etc., should be verified as de-energised prior to and following the use of this device. Blocking Verification Test: If this lockout device cannot be installed in a manner that will hold the appropriate energy isolating devices in a safe or off position, the device shall NOT be used. Application: 1. SPLD type lockout devices are to be permanently attached to the C/B’s cover plate with the screws and/or rivets. Different variations are to be used for different types of C/Bs. Installer/user must ensure use of correct type. 2. With the C/B in ‘OFF’ position, install the SPLD device as shown, using supplied spacers where needed. It may be necessary to disconnect the lockout arm from hinge part during installation. NOTE: If more than one ‘Arm’ is in this pack, use the one with best fit on C/B and discard the others. 3. Insert a padlock with minimum 5mm shackle through both holes. Verify C/B cannot be repositioned to the ‘ON’ position. Attach a Danger tag.
